BAYPOINTE IS NOW AN ACE MEDICAL CENTER

Baypointe Hospital and Medical center is a member of the ACE (Allied Care Experts) Group of Hospitals which is composed of an increasing number of, now around thirty, medical centers, five of which are fully operational and twenty five or more simultaneously emerging in various locations at different stages of completion.

Doctors and investors at Baypointe enjoy portability of privileges and benefits like professional practice and management discounts in all of the currently operational centers (namely ACE MCs - Valenzuela, Baliwag, Pateros and Quezon City) and eventually in the other centers that will open in the future all over the country. Some of these are in the final stages of construction like ACE MCs - Bohol, Tacloban, Cebu, Dumaguete and Iloilo. Soon there will also be ACE MCs in major cities in Mindanao like Zamboanga, General Santos, Dipolog and Butuan.

MANAGEMENT DISCOUNTS FOR INVESTORS

I. STOCKHOLDERS AND SPOUSES

1. Free board and lodging up to suite room accommodation.
2. Fifty percent (50%) discount on the following:

a. Professional fees on house case rates and OPD Consultation
b. X-ray, CT Scan, UTZ, mammography c. Laboratory Procedures
d. EEG, ECG, Tread Mill, 2D Echo e. Physical Therapy

3. No operating room, delivery room and emergency room fees
4.Ten percent (10%) discount for pharmacy, CSR, OR materials
5.For dental services:

a. 50% discount on consultation
b. Thirty percent (30%) discount on Restorative, Preventive, Periodontics, Endodontics, Surgery and Bleaching
c. Ten percent (10%) discount on Prosthodontics procedures

6. For Medical Stockholders (after payment of Privilege to Practice)

a. May hold clinic within the hospital for a maximum of ____ hours
a week paying only the maintenance, water & electric bills and
b. May be included in the decking/rotation of House Cases

II. STOCKHOLDERS' NATURAL PARENTS AND DEPENDENTS

1. Fifty percent (50%) discount on board and lodging up to private room
2. Fifty percent (50%) discount on the following:

a. Professional fees on house case rates and OPD Consultation
b.Operating Room, Delivery Room Fees, Emergency Room Fees

3. Twenty-five percent (25%) discount on the following routines:

a. X-ray, mammography, CT Scan, UTZ
b.Laboratory procedures
c.ECG, EEG, Tread mill, 2D Echo
d.Physical Therapy

4. Ten percent (10%) Discount on Pharmacy, CSR & OR materials
5.For dental services:

a. Free consultation
b. Thirty percent (30%) discount on Restorative, Preventive, Periodontics, Endodontics, Surgery and Bleaching
c. Ten percent (10%) discount on Prosthodontics procedures

III. PORTABILITY (EXTENDED BENEFITS)

1. These benefits, using the schedule of discounts given to dependents (see number II), may be availed of by the principal investor, his/her spouse, dependents and natural parents in all medical facilities that are members of, or affiliated with, the ACE Group of Hospitals such as ACEMCs Valenzuela, Baliwag, Pateros, Quezon City, Bohol, Cebu, Iloilo, Tacloban and Dumaguete.

2. Medical stockholders will have the privilege, subject to pertinent credentialing policies/requirements, to admit and/or manage patients in other ACEMCs.
